INTRODUCTION
Diakonie Katastrophenhilfe (DKH) is a German non-government organisation that renders humanitarian aid in 36 countries across the world. DKH is part of the Evangelisches Werk für Diakonie und Entwicklung e.V. (EWDE), one of the biggest welfare institutions in Germany. DKH works through a global network of partner organisations to support people affected by natural disasters, war, and displacement. The work is guided by the Humanitarian Principles and the Core Humanitarian Standards (CHS).

Special instruction:
All quotations must be submitted on company letterhead, must include Supplier’s legal address, must be stamped, and must be valid for at least 60 calendar days.
Quotation Submission Date: No later than 06 June 2023 at 12 noon in the local time of Aden City Yemen
Submission of Bids:
Bids must be submitted in person to DKH Yemen at October neighborhood, Villa N0. (4), near the Lebanon University, Khor Masker, Aden, Yemen
Quotations submitted in person must be submitted in a sealed envelope and handed to the Logistic Department
Please write “DKH Yemen-RFQ-2023-7” on the top right corner of the sealed envelope with the name of the supplier. The bidders name must also be written on the envelope.
Late bids will not be accepted
Bids to any other DKH office or department will not be accepted
Terms of Payment:
The payment will be made annually upon complete the review and evaluation of the insurance offer and after the agreement of both parties
Specification of requirement:
To provide reasonable, reliable and easily accessible health insurance services to 20 DKH employees, in addition to 3 members (husband/wife, children) 24/7, 7 days per week, and 365 with the respect to:
1. In Patient and Outpatient
2. Optical medical coverage
3. Non-cosmetic dental and skin medical coverage
4. Pregnancy and maternity medical coverage.
5. Accidental medical expenses
6. Accidents medical expenses due to accident related to work.
7. Emergency Cover.
8. Coverage for chronic diseases.
9. Network coverage National/International
10. Insurance Class
11. Life Insurance coverage for DKH employee only
12. Any additional Health insurance services/ optional

يجب تقديم عروض الأسعار المقدمة شخصيًا في مظروف مختوم وتسليمها إلى إدارة اللوجستيات
يرجى كتابة ( “DKH Yemen-RFQ-2023-7 ) في الركن الأيمن العلوي من المغلف المختوم مع كتابة اسم المورد على الظرف، ولن تقبل العطاءات المتأخرة
شروط الدفع:
يتم الدفع سنويًا عند استكمال مراجعة وتقييم عرض التأمين وبعد اتفاق الطرفين
المتطلبات:
توفير خدمات تأمين صحية موثوقة و ممتازة ويمكن الوصول إليها بسهولة 20 موظف بالاضافة الى 3 افراد (الزوج/الزوجة و2 ابناء) على مدار الساعة طوال أيام الأسبوع و 7 أيام في الأسبوع و 365 بما يلي:
1. رعاية الصحية الداخلية و الخارجية
2. التغطية الطبية البصريات
3. التغطية الصحية الاسنان والجلد الغير تجميلية
4. تغطية الصحية للحمل والأمومة.
5. نفقات الصحية عرضية
6. تغطية الحوادث الطبية نتيجة حادث متعلق بالعمل.
7. تغطية حالات الطوارئ.
8. التغطية الامراض المزمنة
9. شبكة التغطية وطنية / دولية
10. فئة التأمين
11. تغطية التأمين على الحياة لموظف DKH فقط
12. أي خدمات تأمين صحي إضافية / اختياري
INSTRUCTIONS TO THE PROSPECTIVE SUPPLIER
A. Form of Quotation
Your quotation must specify and include confirmation of the following:
1. The quotation must be submitted in both English and Arabic languages.
2. Insurance (Provide financial insurance and bank guarantee)
3. Bidder Registration – The Bidder must submit that it is legally registered for business in Yemen and provide a valid business, employer or tax identification document that demonstrates this.
a. A copy of a valid commercial registration.
b. A copy of a valid tax card.
c. A copy of a valid practice license.
4. Quotation Validity – Bidder must specify that the quotation is valid for 60 calendar days.
5. Bidders must complete and submit the attached ‘company profile’. Where possible they should also include references of previous similar work performed for NGOs or other clients and Network lists inside and outside Yemen.
B. Evaluation Factors
DKH shall evaluate the bids as listed below:
1. Bidders, who did not completely submit the required documents for this bid, may not be considered for further evaluation.
2. Bidders need to ensure that they provide all the information required in the two table below
3. The quotation that passes all mandatory requirements and earns the highest points will be determined to offer the best value to DKH. In conducting its evaluation, DKH may seek information from any source it deems appropriate to obtain or validate information regarding the Supplier’s quotation.
4. DKH Yemen may award contracts to more than one bidder
5. Bid must have a validity of 60 days
C. Questions on this RFQ
Questions should be sent by no later than 04 June, 2023 at 3:00 PM in the local time of Aden city Yemen via email to:
procurement.yem@diakonie-katastrophenhilfe.org
Any information given to one Bidder concerning this RFQ will be furnished to all Bidders as an amendment to this RFQ.
D. Contract Award
Bidders should submit their best offer.
E. Receipt of the RFQ
All Bidders shall acknowledge receipt of this RFQ and indicate if they are going to submit a bid.

أ- استمارة عرض السعر
يجب أن يحدد عرض الأسعار الخاص بك ويتضمن تأكيدًا لما يلي:

4. صلاحية عرض الأسعار - يجب على العارض تحديد أن عرض الأسعار صالح لمدة 60 يومًا تقويميًا.
5. يجب على مقدمي العطاءات إكمال وتقديم "الملف التعريفي للشركة" المرفق. حيثما أمكن ، يجب أن تتضمن أيضًا إشارات إلى عمل مماثل سابق تم أداؤه للمنظمات غير الحكومية أو عملاء آخرين والشبكة الطبية في اليمن وخارج اليمن.
ب- عوامل التقييم
تقوم دياكوني بتقييم العطاءات على النحو الوارد أدناه :
1- لا يجوز إجراء المزيد من التقييم لمقدمي العطاءات الذين لم يقدموا المستندات المطلوبة لهذا العطاء بالكامل
2- على مقدمي العطاءات التأكد من توفير كافة المعلومات المطلوبة في الجداول أدناه
3- يتم تحديد عرض الأسعار الذي يجتاز جميع المتطلبات الإلزامية ويحصل على أعلى النقاط لتوفير أفضل قيمة للمنظمة. عند إجراء تقييمها ، قد تسعى دياكوني للحصول على معلومات من أي مصدر تراه مناسبًا للحصول على معلومات أو التحقق من صحتها بخصوص عرض أسعار المورد.
4- يجوز لمنظمة دياكوني أن ترسي عقوداً لأكثر من مقدم عطاء
5- يجب أن يكون العطاء صالحًا لمدة 60 يومًا

سيتم تقديم أي معلومات يتم تقديمها إلى أحد مقدمي العطاءات بخصوص طلب عرض الأسعار هذا إلى جميع مقدمي العطاءات كتعديل على طلب عرض الأسعار هذا
د- إرساء العقد
هـ- استلام عرض السعر
يجب على جميع مقدمي العطاءات الإقرار باستلام طلب عرض الأسعار هذا والإشارة إلى ما إذا كانوا سيقدمون عطاء.
